Don't Submit These to Court
- These are for YOUR records - keep them in a binder or folder
- Don't file these with the court - your lawyer handles that
- Don't give these to DCF unless your attorney says to
- Use them to remember dates, times, and what happened
- Show them to your lawyer when you meet
Why use these? So you don't forget what happened. Court moves fast. DCF says one thing and does another. Write it down while it's fresh.
Forms to Track What's Happening
Print these out. Fill them in. Don't lose them.
📅 Visit Log
Track every visit with your kids - when, where, what you did, how they seemed.
Why: Shows the court you're staying involved.
📞 Communication Log
Write down every call, text, and meeting with DCF and case workers.
Why: It's your word against theirs. Have proof of what was said.
📝 Court Hearing Notes
Bring this to court. Write down EXACTLY what the judge says.
Why: Court goes fast. You'll forget. These notes keep you straight.

Print, cut out, and keep in your wallet.
How to Use Rights Cards
- Print on cardstock (heavier paper) if possible
- Cut along the lines carefully
- Laminate if you have access to a laminator
- Keep in your wallet, on your fridge, or anywhere accessible
- Show to DCF workers if needed: "I want my attorney present"
How to Use These Templates
